Idaho Clinical Laboratory Requirements
Idaho requires more than a federal CLIA certificate: every clinical laboratory must register with the Idaho Department of Health and Welfare (DHW) before accepting specimens for testing, and renew that registration every two years. The registration sits alongside federal CLIA — CLIA-certified labs are excluded from most of Idaho's separate quality-assurance rules, but registration and specified provisions still apply (IDAPA 16.02.06.110).
Scope of this guide
This guide addresses a typical independent commercial clinical laboratory testing human specimens; it is not a legal opinion or final facility-opening checklist. Facility licensure, personnel licensure, hospital/facility rules, out-of-state testing permits, payer enrollment, and specialty rules are separate questions — confirm each with the state authority. Before relying on this guide, verify with the state: ownership/facility type, testing complexity and specialties, whether specimens are collected or tested in-state, whether tests are sent out of state, and whether personnel licenses are required.
State agency
Idaho Department of Health and Welfare (DHW), Idaho Bureau of Laboratories (IBL) — Laboratory Improvement program. IBL regulates over 1,400 CLIA-certified laboratories in Idaho; its clinical lab inspector performs inspections for complex laboratories and monitors proficiency-testing events.
License/registration types
- Idaho clinical laboratory registration — mandatory state registration before the lab accepts specimens for testing (IDAPA 16.02.06.100). This is a registration, not a separate "license" — but it is a prerequisite to operating, and it is separate from the federal CLIA certificate.
- Federal CLIA certificate — still required; Idaho's registration does not replace it.
How to apply
Register with the Department before accepting specimens for testing (IDAPA 16.02.06.100). Each laboratory uses the Department-approved registration form — forms are available upon request from the Department — and submits: the name and location of the laboratory, the name of the laboratory director, the tests performed, and any other information the Department requests to evaluate laboratory performance. Registered laboratories must submit a completed registration form every two years and indicate any changes in laboratory operations.
Forms
State registration
- DHW clinical laboratory registration form — Department-approved form, available upon request from the Department (see the Idaho DHW clinical lab certification page for the program's contact).
- Director Qualification Form — required for Certificate of Compliance or Certificate of Accreditation initial applications or director changes (via the DHW clinical lab certification page).
- Technical Consultant/Technical Supervisor Qualification Form — used to verify qualifications of a technical consultant or technical supervisor candidate for moderate- or high-complexity labs; submitted to LabImprovement@dhw.idaho.gov for review.
Federal CLIA
- Form CMS-116 — CLIA Application for Certification (PDF) — the CLIA certificate application.
- Form CMS-209 — Laboratory Personnel Report (PDF) — keep the personnel roster current for surveyors.
Inspection preparedness — have these ready before the surveyors arrive:
- Form CMS-116 — CLIA Application for Certification (PDF) — surveyors verify the specialties, sites, and director on file match current operations.
- Form CMS-209 — Laboratory Personnel Report (PDF) — current personnel roster; surveyors require it up to date.
- Proof of current Idaho DHW registration and the biennial registration filing.
- Updated Lab Test Menu — the lab's controlled test menu listing every test by specialty; surveyors cross-check it against the certificate.
Fees
No registration fee is established in Idaho Code Chapter 23 (§§ 56-2302–56-2304) or in IDAPA 16.02.06 — the registration is a filing requirement, not a fee-bearing permit (verified 2026-10-20 against the full statute text on legislature.idaho.gov and the current rule text; neither contains any fee provision). The 2025 fee change in HB 198 concerned market-rate testing fees for the state laboratory's own services, not registration. If DHW's Laboratory Improvement program quotes any processing charge, confirm current practice with them before filing (LabImprovement@dhw.idaho.gov, 208-334-0528).
Personnel requirements
Idaho's program sets personnel education, experience, and training requirements, and the state qualification forms document them: the Director Qualification Form (required for Certificate of Compliance/Accreditation initial applications and director changes) and the Technical Consultant/Technical Supervisor Qualification Form for moderate- or high-complexity labs. Federal CLIA personnel standards apply on top of the state registration. See Zilgene's personnel reference pages: Laboratory Director, Technical Supervisor/Consultant, Clinical Consultant, General Supervisor, and Testing Personnel.
Inspection
IBL's clinical lab inspector performs inspections for complex laboratories and monitors proficiency-testing events, under the federal CLIA program. Idaho's rules require adequate facilities and equipment, personnel education/experience/training, proficiency testing, and 2-year record retention — CLIA-certified labs are excluded from most of the separate state QA rules (IDAPA 16.02.06.110) except registration (Section 100) and the specified provisions that still apply (Section 200).
Renewal and changes
Submit a completed registration form every two years and indicate any changes in laboratory operations (IDAPA 16.02.06.100). Report operational changes — director, location, ownership, test menu — to the Department as they occur.
How Idaho differs from federal CLIA
- State registration with DHW is required before accepting specimens — in addition to, not instead of, the CLIA certificate.
- Biennial registration renewal with operational-change reporting.
- Specified state provisions still apply to CLIA-certified labs (IDAPA 16.02.06.110 exclusion covers only the other QA rules).
- State director and technical consultant/supervisor qualification forms document personnel qualifications to the program.
