How Much Does CLIA Certification Cost? Fees and Timelines

One of the first questions labs ask us: what will CLIA certification actually cost? The honest answer is that it depends on your certificate type and test volume — but the ranges are well documented, so you can budget before you apply.

The federal fees. Under the current CMS CLIA certificate fee schedule, a Certificate of Waiver costs about $248 for the two-year period, and a Provider-Performed Microscopy certificate about $297. (CMS updated the fee schedule in its 2024 CLIA fee rule, so if you remember older numbers like $180, they're out of date — always confirm the current figures on the CMS fee schedule before you budget.) Certificates of Compliance and Accreditation scale with your annual test volume and number of specialties: a small lab doing a few thousand tests a year pays a few hundred dollars per cycle, while high-volume labs can owe several thousand.

What the fee doesn't cover. The federal fee is only one line item. Many states charge their own separate licensing or registration fees on top of CLIA, accreditation bodies charge their own membership and survey fees, and you'll have ongoing costs for proficiency testing, quality control materials, and personnel competency programs.

How long it takes. Waiver and PPM certificates can move fast once your application is accepted — often weeks. Certificates of Compliance and Accreditation require an initial survey, so plan on months from application to certificate in hand. Either way, remember that each physical location needs its own certificate, fees are paid every two years, and CMS now handles fee coupons and certificates electronically — keep a monitored email on file so nothing expires by accident.

The bottom line: the certification fee is the smallest part of your startup budget, but it's the one you can't skip. Build the full cost picture — federal fees, state fees, accreditation fees, and QC/PT — before you file your CMS-116, and the financial side of certification will hold no surprises.
