Louisiana Clinical Laboratory Requirements
Louisiana is the mirror image of most states on the personnel side: every person who performs laboratory testing must hold a Louisiana personnel license or certificate — while on the facility side, Louisiana does not require a state facility license (owner-confirmed 2026-10-14; consistent with the Louisiana Department of Health's CLIA program statement, "There are no state laws in Louisiana regulating laboratories. There is only CLIA."). The people side is fully state-licensed; the building side is CLIA-only.
Scope of this guide
This guide addresses a typical independent commercial clinical laboratory testing human specimens; it is not a legal opinion or final facility-opening checklist. Facility licensure, personnel licensure, hospital/facility rules, out-of-state testing permits, payer enrollment, and specialty rules are separate questions — confirm each with the state authority. "Needs state confirmation" means no definitive conclusion was established; it does not mean the state has no additional requirements. Before relying on this guide, verify with the state: ownership/facility type, testing complexity and specialties, whether specimens are collected or tested in-state, whether tests are sent out of state, and whether personnel licenses are required.
State agency
Louisiana State Board of Medical Examiners (LSBME), Clinical Laboratory Personnel Committee — the licensing authority for clinical laboratory personnel under the Louisiana Clinical Laboratory Personnel Law, La. R.S. 37:1311–1329 (mandatory since January 1, 1995, per §1318).
License types
Personnel licenses and certificates issued by LSBME on the Committee's recommendation:
- CLS–Generalist (clinical laboratory scientist-generalist) — license.
- CLS–Specialist — license for practice in one or more laboratory specialties.
- CLS–Technician (clinical laboratory scientist-technician / MLT) — license.
- Laboratory Assistant — license.
- Cytotechnologist / Cytologist — license.
- Phlebotomist — certificate.
- Trainee license — for students working under supervision while in a clinical laboratory program, or returning practitioners who have not practiced for 10 years. Trainees may not report laboratory results.
National certification is the qualifying backbone: licensure applicants must have passed a national examination approved by the Board, with the score report sent directly from the examination agency to LSBME. Laboratory assistant applicants may instead submit a completed competency checklist verified directly by the medical or laboratory director.
How to apply
Apply to LSBME for the license or certificate matching your personnel category. The current application instructions for initial licensure of clinical laboratory personnel spell out the category-specific steps:
- CLS–Generalist, CLS–Specialist, CLS–Technician, Cytotechnologist — licensure fee $65.00 (trainee license also $65.00); national exam score report sent directly from the exam agency to LSBME.
- Laboratory Assistant — licensure fee $40.00 (trainee license also $40.00); competency checklist from the medical/laboratory director, or a national-exam score report.
- Phlebotomist — certification fee $40.00; board-approved phlebotomy training program plus a board-approved or board-administered certification examination.
Fees are non-refundable and paid by credit/debit card (Visa, MasterCard, or Discover only). After LSBME receives and reviews an application, the applicant gets a deficiency report listing anything outstanding — keeping the email address on file current is the applicant's responsibility.
For the facility itself, apply for the CLIA certificate on Form CMS-116 (PDF) — Louisiana requires no separate facility license.
Forms
Application forms
- LSBME clinical laboratory personnel licensure application and instructions — application instructions for initial licensure.
- Form CMS-116 — CLIA Application for Certification (PDF) — the facility's federal certificate.
Inspection preparedness — have these ready before surveyors arrive:
- Form CMS-116 — CLIA Application for Certification (PDF) — surveyors verify the specialties, sites, and director on file match current operations.
- Form CMS-209 — Laboratory Personnel Report (PDF) — current personnel roster; surveyors require it up to date.
- Updated Lab Test Menu — the lab's controlled test menu listing every test by specialty; surveyors cross-check it against the certificate.
- Louisiana-specific readiness: be ready to show current LSBME licenses or certificates for every person performing testing, matched to their category (CLS–Generalist, CLS–Specialist, CLS–Technician, Laboratory Assistant, Cytotechnologist, Phlebotomist).
Fees
Personnel licensure fees (per LSBME): $65 for CLS–Generalist, CLS–Specialist, CLS–Technician, and Cytotechnologist (initial and trainee); $40 for Laboratory Assistant and Phlebotomist (initial and trainee). Non-refundable; credit/debit card only.
Personnel requirements
Louisiana personnel licensure is the personnel requirement: every clinical laboratory scientist, technician, laboratory assistant, cytotechnologist, and phlebotomist must hold the LSBME license or certificate appropriate to their level of expertise — the Louisiana Department of Health's CLIA program confirms that personnel licensure is not administered through CLIA and points applicants to LSBME. Categories and qualifications are set in the Clinical Laboratory Personnel Law (La. R.S. 37:1311–1329) (PDF).
For the federal CLIA role definitions (Laboratory Director, Technical Supervisor/Consultant, Clinical Consultant, General Supervisor, Testing Personnel), see Zilgene's personnel reference pages — and note that in Louisiana, meeting the CLIA role qualifications does not substitute for the state license. Common CLIA questions are answered on the Zilgene FAQ.
Inspection
Facility inspections are federal CLIA surveys (no state facility-licensing inspection exists). Surveyors will, however, verify that testing personnel are qualified — in Louisiana that means checking current LSBME licenses against the personnel roster. The LSBME Clinical Laboratory Personnel Committee also handles licensure discipline (denial, suspension, probation, restriction, revocation).
Renewal and changes
Personnel licenses and certificates must be renewed with LSBME on schedule; confirm the current renewal cycle and continuing-education requirements with the Board before they lapse. Keep the Board's contact email current — deficiency and renewal notices go by email.
How Louisiana differs from federal CLIA
- No state facility license — owner-confirmed 2026-10-14; the facility side is CLIA-only.
- Mandatory individual personnel licensure — CLS–Generalist, CLS–Specialist, CLS–Technician, Laboratory Assistant, Cytotechnologist (licenses) and Phlebotomist (certificate), in effect since 1995.
- National certification examination (score report direct from the exam agency) is the standard qualifying route; laboratory assistants may qualify through a director-verified competency checklist.
- Trainee licenses exist for students and returning practitioners — but trainees may not report results.
- Personnel fees are modest ($40–$65) and non-refundable.
